The Problem

Inefficient and inconsistent patient handoffs at DUHS compromise safety, quality, and care continuity, causing treatment delays, errors, and increased costs. Current EHR tools are labor-intensive, lack nuance, and have limited scalability. Enhancing handoff processes with improved, AI-driven solutions align with Duke Health’s priorities to improve safety, efficiency, and workforce resilience.

Our Solution

We propose expanding the existing Scout AI tool, developed by DIHI, to create a novel AI-driven nurse handoff tool for the co-care model. By integrating additional data sources (flowsheets and medication administration records), Scout will generate a structured handoff summary to support bedside, charge, and virtual nurses.

Anticipated Impact

The AI-driven Nurse Handoff Tool enhances communication, safety, and efficiency across bedside, virtual, and charge nurses. By automating structured handoffs using Scout, it reduces cognitive burden, improves care continuity, and supports Duke’s co-care model. Scalable system-wide, it strengthens workforce resilience and sets a new standard for nursing excellence.
