First Duke Acute Hospital Care at Home patient discharged Friday, April 16, 2021

Duke Raleigh Hospital, in partnership with Duke Institute for Health Innovation and Duke HomeCare & Hospice, is excited to announce the safe discharge of the first Acute Hospital Care at Home (AHCAH) program patient. Hospital at Home is a transformative care delivery model that allows patients to be treated as inpatients in the comfort of their own home.

The enrolled patient consented and received three nights of inpatient-level care ~8 miles north of DRAH, in the safety of their home. This care was supported by Medicare and the CMS Acute Hospital Care at Home waiver, announced on Nov 25, 2020. Duke Home Infusion nurses visited the patient at least twice a day and a Duke Raleigh Hospital provided daily in-person visits. Dr. Vidhya Aroumougame, Dr. Marie Witte, Cynthia Macalino, and Justin Dixon were the first in-home care providers for this patient and must be applauded for their dedication and care – they went above and beyond. Follow-up is scheduled with Duke Primary Care within seven days.
