Problem: HIPAA now requires certain entities to sign an attestation form when requesting patient medical record information if the patient’s records contain reproductive health information. Because Duke Health cannot easily determine which patient records may contain this information, Duke Health Information Management (HIM) will require all requests from these entities to sign the attestation form regardless of presence of reproductive health information. This requirement will increase staff hours needed to complete these requests.

Solution: Create an AI-powered application using AI, LLM, and NLP to identify if a patient’s medical record contains information about STI, substance abuse, mental health stigmata, or pregnancy (STAMP).

Impact: In the last 12 months, approximately 47 HIM staff completed 1,862 requests of information from judicial proceedings, law enforcement, medical examiners/coroners, licensures, state investigators, and responding to subpoenas. The new requirements will add hours of staff effort to communicate and coordinate attestation forms for each of these requests. This solution would reduce the need for the attestation form.
