The idea and implementation of Caremaps is not new. The innovation is that the Breast Cancer Caremaps (BCC) team is creating a customized Caremap that is driven by EHR data and presented in an infographic format to support an ‘at-a-glance’ reference to complex treatment options, decision points and timelines, upon diagnosis. The patient’s treatment plan is translated into a personalized Caremap facilitating understanding and discussion around treatment options during the multi-disciplinary visit. The detailed, personalized, phenotype-driven Caremap will be in a timeline representation of the treatment options and decision points, and timeframes for various treatment options.
The BCC team has made remarkable progress in creating a standardized template to intake data from the various multi-disciplinary clinics and are now exploring ways to deliver the Caremap to patients.
